Office for the Protection of Research Subjects Graduate students conducting any kind of research involving human participation should visit this page to review protocol, obtain certification, and find all required OPRS forms, including the Protocol Proposal form, the Exempt and Excluded Research forms, and The Graduate Student Research Protocol Addendum, and much more.
  15. New Graduate IRB Form (DOC) Students only need this form if they are working on an approved reasearch project for faculty, but are using the research for their own thesis or dissertation.
